- To ensure appropriate understanding and consent for medical procedures (medications, injections, treatments, tests, etc.) that may significantly affect health or life.
- Because minors may lack sufficient ability to understand the risks or side effects of medical treatments.
① Age 16 or older, currently employed, and possesses their own insurance card
We consider the patient capable of informed decision-making and treat them as an adult.
② Age 16 or older, currently receiving regular treatments or prescriptions
If the doctor approves the patient to come alone, they will be given a Consent Form for Unaccompanied Consultation.
Please be sure to bring the signed form at the time of the next visit.
③ If the doctor determines it is difficult for a guardian to accompany the patient due to unavoidable circumstances

- Even if you have a consent form, we may need to contact a guardian regarding the consultation, treatment, or test results.
Please ensure we can reach you when necessary. - In emergencies, the doctor may proceed with necessary treatment or testing without prior guardian consent.
- Depending on the situation, we may decline consultation if we cannot reach the guardian.
- Medical results and fees will be explained directly to the patient who visits.
- We cannot provide any information over the phone about diagnoses or treatment results due to privacy policy.
- If a guardian wishes to hear about the results later, please visit the clinic in person as a follow-up appointment (by reservation or waiting ticket).
