About Making Appointments via Online
Please read this information carefully before booking.
- When making an appointment, please ensure that you arrive at the clinic by the time of your reserved appointment.
(For example, if your appointment is at 4:00 PM, please arrive by 4:00 PM.) - If you arrive after your scheduled time, you may be treated as a walk-in patient and be asked to wait in line.
- If you do not arrive, your appointment will be automatically canceled.
- If the representative is making reservations for multiple people, please create a separate account for each patient and make the reservation for each account individually.
For Minors Seeking a Consultation — Please Read Before Your Visit
★Please complete the online pre-consultation form when making a reservation.
It is possible to make a reservation without the online pre-consultation, but please note that if you do so, you may be called later than those who have completed the online pre-consultation, as you will need to fill out the form upon arrival.
